One of the assignments given to me by my supervisor at work was to write JSA's (Job Safety Analyses) for all classifications within my department. This gentleman would not last long in the real working world. Observed safety violations: 1. Wading into extremely turbulent water; 2. No safety line; 3. Life vest not fastened; 4. Stringing cable together by the hooks and/or tying; 5. If successful reaching the vehicle - surpassing the WLL (Working Load Limit - 20% of Breaking Force) for wire rope (cable). The vehicle is holding/trapping approximately 35 cubic feet of water (2,187.5 lbs). The left side of the vehicle (approx. 80 sq ft of surface area) is being acted upon by an extreme current. Even if this is a 2 1/2 version truck the combined weight and forces would reach 80% of the Breaking Force for NEW 1/2 inch wire rope. Observation revealed loose strands being removed from the cable attached to the tow vehicle. This was not a good scenario. |
